As strictly a bolt on there are none that I know of (but then again I don't really have much expirence with a cb500t).

The two popular wheels/rear ends to use for disc brake conversions in the 1970s were 1975-76 cb750F and 77-79 GS750 (spoke rims - the GS also came with mags and the cb750F had mags in 77-78). These are still antiquated discs by modern standards but their overall rim width probably means they will fit inside your frame. The main issues you will need to deal with are chain line, wheel spacing, and axle diameter. There is a lot of custom stuff involved in this so make friends with a welder and a machinist.

As long as you are going to to massive modifications you might as well upgrade to modern discs and wheels. I personally don't think any 70s disc is gonna stop as well as your drum rear so if weight is the only motivating factor for the swap I'd look at an early ex500 wheel. Kinda modern three spoke mags, lighter, and a much better drum. Once again you will have to deal with chain line, wheel spacing, and axle diameter. Anything more modern and the rim is likely to be way to big.

go to the bike salvage yard with a tape measurer and explore. That is how I put fzr wheels and suspension on my h1
